Steps to Follow:

1. Fill out the form and click Submit
2. Add Dryer Repair Service to Cart
3. Add Return Shipping Label to Cart (if you want the dryer shipped back to you)
4. Repeat steps 1-3 for each dryer (if sending multiple dryers)

Shipping Instructions:

• Please do not send any accessories with your dryer, including:
  Nozzles
  Diffusers
  Silencers
  Any other attachments or accessories
• Ship dryer to:
  Parlux Service Center, 1956 S La Cienega Blvd, Los Angeles, CA 90034
• Please add a note with:
  Your Name
  Order Number
  inside the package

Important Notice:

• Parlux Repair Center is not liable for any loss or damage to your dryer during transit to our repair center.
• Please get signature confirmation from the carrier to ensure your dryer is protected against loss during transit to the repair center.
• Note: A separate form must be filled out for each dryer
